Background The prioritization protocols for accessing adult critical care in the extreme pandemic context contain tiebreaker criteria to facilitate decision-making in the allocation of resources between patients with a similar survival prognosis. Besides being controversial, little is known about the public acceptability of these tiebreakers. In order to better understand the public opinion, Quebec and Ontario’s protocols were presented to the public in a democratic deliberation during the summer of 2022. Objectives (1) To explore the perspectives of Quebec and Ontario (...) citizens regarding tiebreakers, identifying the most acceptable ones and their underlying values. (2) To analyze these results considering other public consultations held during the pandemic on these criteria. Methods This was an exploratory qualitative study. The design involved an online democratic deliberation that took place over two days, simultaneously in Quebec and Ontario. Public participants were selected from a community sample which excluded healthcare workers. Participants were first presented the essential components of prioritization protocols and their related issues (training session day 1). They subsequently deliberated on the acceptability of these criteria (deliberation session day 2). The deliberation was then subject to thematic analysis. Results A total of 47 participants from the provinces of Quebec (n = 20) and Ontario (n = 27) took part in the online deliberation. A diverse audience participated excluding members of the healthcare workforce. Four themes were identified: (1) Priority to young patients - the life cycle - a preferred tiebreaker; (2) Randomization - a tiebreaker of last resort; (3) Multiplier effect of most exposed healthcare workers - a median acceptability tiebreaker, and (4) Social value – a less acceptable tiebreaker. Conclusion Life cycle was the preferred tiebreaker as this criterion respects intergenerational equity, which was considered relevant when allocating scarce resources to adult patients in a context of extreme pandemic. Priority to young patients is in line with other consultations conducted around the world. Additional studies are needed to further investigate the public acceptability of tiebreaker criteria. In face of the multiple controversies surrounding the DSM process in general and the development of DSM-5 in particular, we have organized a discussion around what we consider six essential questions in further work on the DSM. The six questions involve: 1) the nature of a mental disorder; 2) the definition of mental disorder; 3) the issue of whether, in the current state of psychiatric science, DSM-5 should assume a cautious, conservative posture or an assertive, transformative posture; 4) the role (...) of pragmatic considerations in the construction of DSM-5; 5) the issue of utility of the DSM – whether DSM-III and IV have been designed more for clinicians or researchers, and how this conflict should be dealt with in the new manual; and 6) the possibility and advisability, given all the problems with DSM-III and IV, of designing a different diagnostic system. Part 1 of this article took up the first two questions. Part 2 took up the second two questions. Part 3 now deals with Questions 5 & 6. Question 5 confronts the issue of utility, whether the manual design of DSM-III and IV favors clinicians or researchers, and what that means for DSM-5. Our final question, Question 6, takes up a concluding issue, whether the acknowledged problems with the earlier DSMs warrants a significant overhaul of DSM-5 and future manuals. Pancreatitis is a complex, progressively destructive inflammatory disorder. Alcohol was long thought to be the primary causative agent, but genetic contributions have been of interest since the discovery that rare PRSS1, CFTR and SPINK1 variants were associated with pancreatitis risk. We now report two associations at genome-wide significance identified and replicated at PRSS1-PRSS2 and X-linked CLDN2 through a two-stage genome-wide study. The PRSS1 variant likely affects disease susceptibility by altering expression of the primary trypsinogen gene. The CLDN2 risk allele is (...) associated with atypical localization of claudin-2 in pancreatic acinar cells. The homozygous CLDN2 genotype confers the greatest risk, and its alleles interact with alcohol consumption to amplify risk. In the conclusion to this multi-part article I first review the discussions carried out around the six essential questions in psychiatric diagnosis – the position taken by Allen Frances on each question, the commentaries on the respective question along with Frances’ responses to the commentaries, and my own view of the multiple discussions. In this review I emphasize that the core question is the first – what is the nature of psychiatric illness – and that in some manner all further (...) questions follow from the first. Following this review I attempt to move the discussion forward, addressing the first question from the perspectives of natural kind analysis and complexity analysis. His theological output even prior to becoming pope (...) is staggering. It is thus fitting that an edited volume examine his theological oeuvre; there is a need for many more such volumes. The essays collected in this book were originally presented at a conference on the theology of Pope Benedict at the University of Notre Dame. Each contribution brings something insightful and unique to the volume. The book is divided into three sections: “The Dynamic of Advent,” encompassing chapters 2–5; “Caritas in Veritate,” encompassing chapters 6–7; and “God is Love,” encompassing the remaining chapters 8–11. The initial chapter-length introduction and the first chapter fall outside of this division because in many ways they set the stage for the entire volume.John Cavadini’s introduction (1–20) could serve as a useful overview of the theological work of Pope Benedict. Cavadini provides a preview of each chapter but also underscores the significance of Benedict’s theological work. He emphasizes how “one of Benedict’s major achievements is the demonstration of Augustine’s original insight into the unity of the theological tasks of understanding and of engendering understanding” (3).Cyril O’Regan’s very fine essay, “Benedict the Augustinian” (21–60), examines the Augustinian nature of Benedict’s theology. O’Regan maintains, “throughout his career Benedict not only turns again and again to Augustine as his theological model, but also... in Benedict’s self-understanding[,] the basic figuration of his theology is Augustinian” (22). This Augustinian texture to Benedict’s theology involves, among other things, his “real familiarity with and admiration” for “the classic” works of Augustine, his use of Augustine’s “theological style,” his use of shared themes, and the manner in which he consciously “articulates substantive theological positions” typical of Augustine. Moreover, O’Regan points to “Benedict’s sense that he is living in a time of crisis and senescence similar to that of Augustine” (21–22). Some of the similarities O’Regan observes in the theological work of Augustine and Benedict pertain to eschatology, the relationship between faith and reason, biblical interpretation, culture, the role of the liturgy, and the centrality of prayer. O’Regan’s essay is undoubtedly the best piece I have read on the importance of Augustine for Benedict.The thesis of Peter Casarella’s “Culture and Conscience in the Thought of Joseph Ratzinger/Pope Benedict XVI” (63–86) is that for Benedict, “the witness to truth is the key to seeing how culture and conscience are necessarily intertwined” (64). Casarella initiates his investigation with the pope’s critique of relativism. The first major section examines what he terms Benedict’s “theology of culture.” For Benedict, this has a moral dimension, and thus [End Page 493] Caserella spends the entire second portion of his essay looking at conscience and its formation in the context of culture. This is a very helpful essay that makes an important contribution to Benedict’s thought as it relates to moral theology.The late Fr. Edward Oakes’s contribution, “Resolving the Relativity Paradox: Pope Benedict XVI and the Challenge of Christological Relativism” (87–113), explores Benedict’s problem with relativism, but homes in on relativism in the context of Christology. Oakes argues, “the challenge of relativism cannot be met unless it is done first and foremost in Christological terms” (93), and he thinks Benedict does precisely this. Oakes’s overview of Ernst Troeltsch’s discussion of historical criticism as corrosive to Christology is enlightening:what most undermines Christocentrism is the historical-critical method, not so much because of the results arising from that method as from its very use.... Generalization is a fundamental problem solved by every cognitive system in essentially every domain. Although it is known that how people generalize varies in complex ways depending on the context or domain, it is an open question how people learn the appropriate way to generalize for a new context. To understand this capability, we cast the problem of learning how to generalize as a problem of learning the appropriate hypothesis space for generalization. We propose a normative mathematical framework for learning (...) how to generalize by learning inductive biases for which properties are relevant for generalization in a domain from the statistical structure of features and concepts observed in that domain. More formally, the framework predicts that an ideal learner should learn to generalize by either taking the weighted average of the results of generalizing according to each hypothesis space, with weights given by how well each hypothesis space fits the previously observed concepts, or by using the most likely hypothesis space. We compare the predictions of this framework to human generalization behavior with three experiments in one perceptual (rectangles) and two conceptual (animals and numbers) domains. FASTIGGI St. Edward's University Austin, Texas Introduction: Christian-Muslim Dialogue and the Incarnation THE TWO largest religions in the world, Christianity and Islam cannot help but encounter each other. In the last two decades, several important steps have been made by Catholics, Protestants and Orthodox Christians to engage in meaningful dialogue with members of the Islamic faith.1 While sincerity, mutual (...) respect and good will are all in evidence in these efforts to dialogue, it also is clear that an authentic theological discussion between Christians and Muslims cannot help but arouse a certain degree of tension over the Christian doctrine of the Incarnation. In my own experience in dialogue with Muslims,2 there seems to emerge a point in which discussion of Jesus the prophet vs. Jesus the Incarnate Word of God will inevitably take place. Such discussions can prove to be stimulating and educational. FASTIGGI the two traditions. As Georges Anawati, a Catholic Islamicist; notes : " As a starting point, dialogue requires respect for the identity of the other. It does not ignore obvious differences... for to do so would be of no use for ·either friendship or truth." 3 A Muslim scholar, Professor Mohammed Taibi of the University of Tunis, likewise remarks that " the more one is firm about the classical aspects of divergence, the better the contacts between Christians and Muslims." 4 At issue in the Christian-Muslim discussion of the Incarnation is the unique character of each faith. In Islam, there is the absolute affirmation of divine unity (tawhid) which necessarily condemns any attempt to associate something other than God with God (shirk). Christianity, though, has as one of its central tenets the affirmation that the uncreated Word of God (ho logos) became flesh (sarx) in Jesus (John 1 :14). Islam, therefore, presents a unique challenge to the Christian theologian. Is it possible to formulate a Christology which can affirm the Islamic principle of tawhid and evade the accusation of shirk without compromising the core of the Christian message? This is the question this article hopes to address. One suggestion for Christians in dialogue with Muslims is given by Hans Kiing in his book Christianity and the World Religions.5 Kiing believes that Christians would do best to go back to the mindset of the original Jewish Christians who possessed an understanding of Jesus which is much more compatible with the Qur'anic view of Jesus as the "servant of God." 6 While Kiing's suggestion is helpful for Christians who are looking for an analogy to the Islamic view of Jesus, I find it problematical for several reasons. Alvin Plantinga’s solution to J. L. Mackie’s logical problem of evil invokes possible-worlds metaphysics. There are reasons for thinking that the solution is, at least, problematic. Difficulties emerge in the attempts to answer four related questions. Can God’s necessary existence, understood in terms of possible-world metaphysics, make God’s actual existence impossible to explain? Can an omniscient being with knowledge of the contents of every possible world prove ignorant of the consequences of his creative acts? Can an immoral action performed by (...) an agent suffering from “transworld depravity” also be free in the libertarian sense? Does the possible-worlds interpretation of libertarian freedom generate a vicious infinite regress? Special focus is on the possibility, advanced by Plantinga, that there are possible worlds that even an omnipotent being cannot create. Plantinga’s views are contrasted with those of Thomas Aquinas. Since its more or less formal beginning in the sixteenth century as a reconstruction of ancient drama, opera as an art form has been controversial. The received understanding—emphasized by the genre's founders and in periodic efforts at reforming the standards of composition and production—is that opera is musical drama. In his book Opera (...) as Drama, Joseph Kerman reasserted this view as an antidote to what he called "flabby relativism" and in order to be able to determine a set of values that could serve as a basis for opera criticism.1 He argues that while opera is "excellently its own art form," it is one where the story serves as the central framework in terms of which the particularities of the words and music are structured.2 Given a story, the librettist and the composer work to express the narrative dramatically, using words and music to convey not only the events of the story but the emotions bound up in the story. For Kerman, in a successful opera the words express the narrative and the music enriches it by providing the emotional aspect. One can then evaluate an opera in light of the drama expressed in the relation of the narrative to the words and music. Disconnection between the words and music undermine the value of the opera, even if the words adequately express the narrative and the music adequately expresses some emotion.Philosopher Bernard Williams, in his collection On Opera, affirms Kerman's general conception of opera and adds to it two important factors. First, while the words and music are essential to the drama presented, operas are also distinct in that they are staged and not concert pieces. In this case, the details of the staging also contribute to the success of a particular opera (though the variation in staging usually falls out of discussions by [End Page 74] Kerman and Williams of particular operas). Second, Williams makes clear what is implicit in Kerman: "It is a fallacy to argue," Williams says, "that since, in a musical drama, music obviously provides the music, so the words must provide the drama."3 The drama is a product of both the words and the music in relation to the narrative whose meaning provides the frame on which these hang. Opera criticism, from this perspective, focuses on the specific ways in which the text of a given opera expresses or fails to express the drama of the story. Here, the words, since they present the story, are taken as primary. Williams quoted Calzabigi, a librettist and reformer associated with Gluck: "The music has no other function than to express what arises from the words, which are therefore neither smothered by notes nor used to lengthen the spectacle unduly, because it is ridiculous to prolong the sentence 'I love you' (for instance) with a hundred notes when nature has restricted it to three."4In contrast to Kerman's view of opera as drama, philosopher Peter Kivy, in his book Osmin's Rage, proposes a view of opera as music. Despite Kivy's claim that this notion serves as a more or less equivalent alternative to Kerman's, in fact it radically reorganizes how one thinks about opera and how one evaluates particular operas. Kivy's alternative is in part the result of a reading of the history of opera as an art form that began, in effect, as the answer to a philosophical problem: the relation of emotions to dramatic texts. The founders of opera, the Florentine Camerata, asked how, given a particular story—Antigone, Oedipus, or Orpheus, for example—does telling the story express the emotions that are central to the meaning of the story? How does telling the story go from "just the facts" to the felt meaning of tragedy? The answer for the Florentine Camerata was found in their understanding of ancient Greek tragedies, where they concluded that the problem was solved by singing the words. By singing, they argued, the Greeks were able to make the emotional content of the words accessible to the audience.5... (shrink)

In early 2017, Nevada amended its Uniform Determination of Death Act, in order to clarify the neurologic criteria for the determination of death. The amendments stipulate that a determination of death is a clinical decision that does not require familial consent and that the appropriate standard for determining neurologic death is the American Academy of Neurology’s guidelines. Once a physician makes such a determination of death, the Nevada amendments require the withdrawal of life-sustaining treatment within twenty-four hours with limited exceptions. (...) Neurologists have generally supported Nevada’s amendments for clarifying the diagnostic standard and limiting the ability of family members to challenge it. However, it is more appropriate to view the Nevada amendments with concern. Even though the primary purpose of the UDDA is to ensure that all functions of a person’s entire brain have ceased, the AAN guidelines do not accurately assess this. In addition, by characterizing the determination of death as solely a clinical decision, the Nevada legislature has improperly ignored the doctrine of informed consent, as well as the beliefs of particular faiths and cultures that reject brain death. Rather than resolving controversies regarding brain death determinations, the Nevada amendments may instead instigate numerous constitutional challenges. (shrink)

In a magisterial book-length study, Professor E. Christian Brugger concludes that the canons of the Council of Trent, given the beliefs and intentions of its participants, provide “a dogmatic definition of the absolute indissolubility of marriage as a truth of divine revelation” (original italics). The present concern is whether Brugger’s arguments support this conclusion. Also subject to scrutiny are the relevance, plausibility, and consistency of the conciliar thinking on which his arguments are premised. It will be argued that Brugger’s conclusion (...) is unwarranted, leaving the question of divorce and remarriage an open one. (shrink)

Wooo History Making History: The New Historicism in American Religious Thought. By WILLIAM DEAN. Albany: State University of New York Press, 1988. Pp. xiv + 175. $57.50 (hardcover) ; $18.95 (paper). This hook sets out to explicate, not historicism generally, hut a particular local and American variety thereof. Indeed one of Dean's major points concerns the importance of " local knowledge." He is addressing readers located, for better or worse, in American institutions, in a time and space shaped by American experience. These, he argues, need to cultivate a distinctively American way of knowing in philosophy and religion, a way of knowing that will be historicist, pragmatic, and empirical in character; adopting it would require important revisions in how we conceive and practice theology. Like all good historicists, Dean presents his argument by telling a story, creating a narrative context for the understanding of ideas. The story is most immediately about a group of contemporary thinkers that includes secular philosophers (Richard Bernstein, Nelson Goodman, Hilary Putnam, Richard Rorty), philosophers of religion and ethics (Jeffrey Stout, Corne! West), theologians (Gordon Kaufman, Mark C. Taylor), and a lone literary critic, Frank Lentriccia. By treating these writers together as " the new American historicists," Dean hopes to BOOK REVIEWS 541 show that they exemplify a distinctive style of thought that is both locally valid (because rooted in American traditions and practices) and religiously interesting. At the same time, the new historicists suffer from an ironic case of intellectual amnesia. While they consistently stress the inevitability of tradition and context in shaping thought, these writers often seem oblivious to the uniquely American tradition in which they stand. Thus Dean reaches back to recover elements of an older American historicism with roots in pragmatism, religious naturalism, and the liberal Protestantism of the prewar " Chicago School." The thought of these illustrious precursors, he argues, can help save today's historicists from the formalism that is their constant temptation. But what is historicism? Dean makes an emphatic distinction be· tween the historicism that grows out of the German idealist tradition and the historicism that " can be traced to the classical era in Ameri· can philosophy," e.g., to the thought of the pragmatists (p. 2). When Dean speaks of the older, idealist-influenced historicism he does not mean simply Ernst Troeltsch's famous proposal for a form of cultural and religious relativism; he is painting with a much broader brush. " Continental " historicism means for him something more like the idealist conviction concerning the " historicity " of the subject. After Kant, we do not simply apprehend the world, we construe it; and ever since Kant we have become more and more aware that we do so from particular, contingent perspectives. " Historicism" in this wide sense might be taken as a thesis about the inevitability of hermeneutics. But belief in the necessity of interpretation or hermeneutics is com· patible with affirming the reality of certain universal anthropological structures, e.g., of "reason" or "subjectivity" or "understanding". Pursuing a suggestion made by Christopher Stead in his book Divine Substance and employing distinctions made by Gottlob Frege in his article “Concept and Object,” it becomes possible to answer a common charge against Trinitarian Theism: its alleged inconsistency in claiming that, while there is only one God, there are also three “persons,” each rightly named “God.” The argument advanced, while supporting the logical coherence of traditional Trinitarian Theism, also defends the orthodoxy of the controversial “Social Trinitarianism” associated with Richard (...) of Saint Victor.
